EuroHeartPath aims to transform cardiovascular care in Europe by improving the entire care pathway—from early detection to prevention—for four key conditions: heart failure, atrial fibrillation, ventricular fibrillation, and coronary disease. The project will map current practices in 45 hospitals across 15 countries and conduct 18 pathfinder studies to test the integration of innovations such as AI, digital tools, point-of-care testing, and robotics. By involving patients, cardiologists, and family doctors, EuroHeartPath ensures that solutions are practical and meet real needs. The ultimate goal is to identify and promote best practices that can improve outcomes for patients and strengthen health systems.
EUPHA’s role as a partner
EUPHA will support improvements in care delivery by sharing best practices across countries, helping to ensure patient data is used ethically and safely, and contributing to the planning of how the project’s results can be sustained and used in the long term. EUPHA brings the expertise from within the EUPHA Chronic Diseases section and EUPHA project management to this ambitious initiative.
