Poorly integrated health and social care services and a lack of trans-sectoral coordination models are major health system problems. Many healthcare workers are faced with the challenges of closing existing institutional gaps and mitigating policy failures in their day-to-day practices. They are continuously walking the walk between the health and social care sectors, thus playing an important role in building bridges for better integration and coordination of care. What to learn from these experiences on the challenges, chances and needs of trans-sectoral work? This webinar addresses these questions with a focus on the health workforce in two major areas: community care and long-term care.
