Results of Joint Action Chrodis (2014-2017)

The Joint Action CHRODIS-PLUS has been helping countries address the growing burden of chronic diseases since 2017. To test tools and good practices in addressing chronic diseases, it has implemented 21 pilot projects. To promote long-term sustainability and integration of good practices into policy, 16 policy dialogues have been organised. The Joint Action CHRODIS-PLUS brings together 50 partners, including EuroHealthNet, from 21 European countries. It is co-funded by the European Commission health programme.
