In the context of increasing use of integrated care pathway, often supported by technology (to support timely communication and data sharing),

this presentation will provide an overview of a structured way (a framework) to support the implementation of integrated care.

The framework was developed as part of an international EU funded randomised Controlled Trial called.

The framework combines ‘Realist Research’, the ‘Consolidated Framework for Implementation Research’, and

the ‘Non-Adoption, Abandonment, and challenges to Scale-up, Spread, and Sustainability’ as an evidenced-based but practical was to guide

clinicians and research on the steps, processes, facilitators, and challenges to implementing integrated care.
